Stability doesn't only happen in an office. Our Community Support Program meets you where you are — at home, in your community, on your schedule — and stays with you for the long stretch, not just the hard week.

Community Support is for Iowans ages 5 and older living with an ongoing mental health condition who need more than an occasional appointment. Maybe getting to an office is hard. Maybe the paperwork has piled up. Maybe nothing is exactly a crisis, but nothing is steady, either. That's the gap we're built for.
Together, you'll work on whatever actually makes life feel more manageable: keeping medications on track, sorting through benefits paperwork, setting up appointments, connecting with a doctor or therapist, building routines that hold.
This isn't a short program. We stay with you for as long as you need us — months, sometimes years — because the things that get someone steady don't fix themselves in a single visit. You don't have to have it all figured out before you call.

A familiar face who comes to where you live, on a regular schedule, so you don't have to figure out how to get to one more appointment.
Help keeping medications on track, getting refills filled, and creating follow-up care to ensure all your health needs are met.
A real person to sit beside you through SSI, Medicaid, housing applications, and the forms that should be simple but aren't. We translate the small print.
Help finding the doctor, the peer group, the food pantry, the ride. We know Iowa's resources and we know the people behind them.
This is your program. We don't ask for perfect — we ask for honest.
We believe support works best when it fits your life. We'll take time to understand your goals, strengths, challenges, and what matters most to you, so we can focus on the supports that are meaningful and helpful right now.
We keep a regular schedule of visits. Some weeks that goes smoothly; some weeks it doesn't. We work with what's real, not what's ideal.
You set the goals. We bring the tools, the rides, the paperwork, and the steady company. This isn't something we do to you — it's something we do with you.

Call 515-642-7070. You can be calling for yourself, or for someone you love. Family members, doctors, and other providers all make this call every day.
Within a few days, we'll set up a first conversation — at our office, in your home, or over the phone, whichever is easier. This is where we start to learn your story.
From there, we map out what regular support looks like — how often we visit, what we focus on first, who else needs to be in the loop. You drive the plan.
Community Support is available to children, adolescents, and adults throughout much of Central Iowa who are seeking extra support to maintain stability and wellness in their everyday lives. Service availability may vary by location, but we're happy to discuss your needs and determine whether we can serve your area.
